For some transmen, testosterone therapy and top surgery is not enough to make their outer appearance reflect how they feel inside. Bottom surgery, also known as genital confirmation surgery is often sought out by transmen as the final gender confirmation surgery. This is a complex surgery requiring knowledge and skills that only a handful of surgeons have in the world.
A phalloplasty was pioneered by Dr. Harold Gillies in 1964. At the time, it was a revolutionary approach and in the beginning stages. Since that time, the surgery has been continouously refined and improved.
Dr. Peter Raphael with the International Center for Transgender Care created a radical new improvement over the previous phalloplasty surgery and coined it the “Centurion.”
Dr. Raphael first performed the Centurion surgery in 2002. This surgery is performed along with a hysterectomy and a vaginoplasty. The surgery is considered a safe surgery that takes approximately 2 ½ hours. The clitoris is released and the erectile tissue of the clitoris is used as the foundation of the new neo-phallus erection after surgery. The labia majora is used to create the scrotal sac where testicular implants are used. A neo-uretha is created using available tissue and begins at the native uretha opening.
